2015 National Bankruptcy Forms Modernization

Please be advised that most official bankruptcy forms are scheduled to be replaced with substantially revised, reformatted, and renumbered versions effective December 1, 2015. Use of the revised official forms will begin on that date. Attorneys should contact their software vendors for more information. The revised forms and other related information can be found on the U.S. Courts website at http://www.uscourts.gov/rules-policies/pending-rules-amendments/pending-changes-bankruptcy-forms.

IMPORTANT INFORMATION ON FILING REQUIREMENTS in the U.S. Bankruptcy Court:

  • use of the new OFFICIAL bankruptcy forms WILL BE REQUIRED for all cases commenced on and after December 1, 2015
  • use of the new OFFICIAL bankruptcy forms WILL BE REQUIRED for any amendments to the petition, schedules and statements AND conversions filed December 1, 2015 and after in cases commenced prior to December 1, 2015

Reminder:

Bankruptcy Courts across the US will perform required system maintenance related to the revision of official forms. Many states are reporting their systems will be down after the close of business on Monday Nov 30 until some time in the early morning on Tuesday Dec 1, at which time access will be restored and CM/ECF will be available for use with the new Bankruptcy forms.
